Healthify has partnered with Fortis Hospitals to introduce a hospital based weight management program that combines clinical supervision with digital health tracking. The program has been launched at Fortis Hospital Mohali and will expand to other centres in phases based on outcomes and learnings.
The initiative comes at a time when India is seeing a rise in obesity, metabolic conditions and lifestyle related health concerns. The program is designed to address gaps in long term weight management by focusing not just on weight loss, but on sustained behaviour change and overall health improvement.
The model begins with clinical evaluation by endocrinologists and specialists, followed by personalised care pathways based on individual health profiles. These may include nutrition planning, activity recommendations, lifestyle interventions and ongoing monitoring, with additional medical support where required.
Healthify’s digital platform enables continuous tracking of food intake, physical activity, sleep and key health indicators between consultations. This integrated approach connects patients, doctors and coaches, allowing for ongoing adjustments to care plans and improving engagement over time. By combining hospital led care with digital support, the program aims to improve adherence and deliver more consistent long term outcomes in weight management.
